The Early Years Multi-Agency Group (EYMAG) is a multi-professional forum. The Group is made up of local authority and health professionals, who meet monthly to discuss children 0-4 (pre-school) with significant/complex additional needs.
The EYMAG is the single pathway to accessing Child Development and Portage Services and all professionals are responsible for referrals into the group.
A summary of the child’s needs is presented to the Group by the Chairperson.
The Group considers the view of the parents / carers (as outlined on referral form).
Information from all involved agencies is shared, and discussion takes place.
Actions are agreed, and Action Notes are sent to parents and involved services detailing the outcome of the discussions.
The Group will always consider whether or not initiating a Common Assessment Framework (CAF) would be helpful for a more holistic supportive approach.

Eligibility criteria and restrictions : Referrals to the EYMAG must be made by a professional supporting the child, not an Early Years setting. A child will only be discussed when signed parental agreement has been obtained; no action will take place without consent.

Via CAMHS (Child and Adolescent Mental Health Services)
Via GP or Health Professional
Via Children’s Social Care
Additional Referral Information: All professionals (where involved) are responsible for referring into the group, by completing a parental agreement form.
Waiting List: All appropriate referrals will be accepted, and where possible will be discussed at the next meeting (which occur monthly). Agendas are sent out two weeks prior to each meeting.

Additional Method(s) of Service Delivery: A multi-agency group meets to discuss a child’s needs, who then identify services that can offer relevant support. Following discussion Action Notes are created and a lead professional is appointed who ensures feedback is provided to the family.

Additional Local Authority Areas: Liaisons with other authorites will occur if a Cheshire East child attends a setting outside Cheshire East.
